We are asking that parents help the students prepare for this testing by making sure they get 8 to 9 hours of sleep per night.  A good breakfast on these morning will be beneficial for your child also.  If at all possible, please avoid making doctor appointments during this testing time.  Your help and cooperation on this matter will greatly aid your child.

Dear Parents,

Ohio State Law requires Boards of Education to adopt formal policy regarding the administration of medication to students at school.

The law gives schools option of not administrating medication at school under any conditions, or to do so under strict guidelines established by the law.

The Delphos City Schools policy does permit the administration of medication at school in accordance with the attached guidelines.  We realize the inconvenience not doing so would afford many parents.

Students will not be permitted to have medication, prescription or non-prescription, on their person or in their desk.  All medications MUST be stored in the office.  Only exception would be inhalers, which the student may keep with them if needed.

For prescription drugs, both sides of the pink form (doctor's and parent's instructions) must be completed, signed, and turned into the office before the school, or the student, can administer the medication.  Please send both form and medication to school if it is required to be given during school hours.

For non-prescription drugs, the yellow form (parent's instructions) must be completed, signed, and turned into the office before the school, or the student, can administer the medication.  Please send form and medication (such as Tylenol, Tums, etc.) in original container with your child's name on it to school if it is needed during school hours.

Click on the underlined phrases in the parenthesis above to go to the correct forms.  Additional forms may be obtained through our office.

Your cooperation in following the administration of medication within school guidelines will be appreciated.  We are sorry for any inconvenience, but we must follow state laws.

Please call if you have any concerns.

Sincerely,

Mark Fuerst
