1)    Emergency Medical Forms-

2)    Medication Authorization Form - K-12

It is recommended that medication be given at home, but if this is not possible, school personnel (school nurse or persons designated by school policy) may administer medications if the following requirements are met:

·        This form must be completed for each medication whether it is for a prescription or over-the-counter medication such as Tylenol. Parental signature is required for both prescribed and non-prescribed medications. A physician's signature is required for all prescription medication. Aspirin is not recommended for school age children and will not be given at school unless ordered by a physician.

·        Medication must come from home along with this completed form the day the child is to start taking the medication. A signed note from the physician is acceptable until the parent can get the proper form. You must provide the medication.

·        The medication is delivered to the school in the original labeled container in which it was dispensed by the prescribing physician, licensed pharmacist, or manufacturer. Give to secretary or school nurse.

·        A written physician's statement or new form is needed if any of the required medication information should change.

3)    Authorization for Student Possession & Use of An Asthma Inhaler- complete this form if your child needs to carry an inhaler at school.  Turn completed form into the school nurse at beginning of school year.

4)    Authorization for Student Possession & Use of An Epinephrine Autoinjector- complete this form if your child need to keep an Epipen or Epinephrine Autoinjector at school. Turn completed form into the school nurse at beginning of each school year.

5)    School Policy on Communicable Diseases- find out when your child is considered contagious and should be excluded from school.

6)    Immunization Requirements for K-12 School Attendance- check the Ohio Department of Health’s requirements for immunizations and school entry for Kindergarteners and any new students to the district.

7)    Recommendation for students over the age of 10 to update Tetanus Status with a Tdap shot

·       A Tetanus-Diphtheria-Pertussis (Tdap) shot is recommended to all MS & HS students if they have not had a tetanus booster in the last five years. Immunity to these bacterial diseases has most likely faded over time since most MS & HS students have not had a booster for tetanus, diphtheria, and pertussis since kindergarten. Once your child receives the shot, please send in documentation to the school nurse so the health record can be updated. Attached is the Tdap Vaccine Information Sheet from the CDC.

8)    Birth Certificate Requirement- a copy of the official birth certificate issued from the health department in the county where the child was born is required for all Kindergarteners and any student new to the district in grades 1st-12th. Hospital birth certificates are no longer acceptable.

9)    Eye Exam Requirements for Special Education Students

Amended Substitute House Bill 95 passed in 2003 requires students initially identified with disabilities to have a comprehensive eye exam from an eye doctor. If the student had an eye exam during the previous nine months, this requirement is waived. If you do not have insurance, it will be provided for free. See the following attached forms:

10) Sports Physical Forms- required for all student athletes in grades 7-12th playing sports in school athletics.  After physical is completed by a physician and all forms signed by the student and a parent, please turn all forms into the office or athletic director. Please print off the following forms:  

11)  Mobile Dentist Application- If interested in having your child seen by the mobile dentist from the Dental Center of Northwest Ohio during the school year at Spencerville Schools, please contact the school nurse for forms. They accept most private insurance, Medicaid, or will bill for dental services for children under 18 years old at $25/visit.

Adult fees are based on the Medicaid reimbursement rates. Proof of income (such as two recent pay check stubs) will be required on the first visit to the Dental Clinic and annually thereafter to receive the reduced fixed rate fee for individuals living at or below 300% of the Federal poverty level.
