ATTENTION ALL LIBERTY-BENTON ATHLETES!!!! Sports physicals will be given on Saturday, June 2, 2007 at: Northwest Ohio Orthopedics & Sports Medicine(NWO) 7595 CR 236, Findlay Physical forms can picked be downloaded or picked up in the Middle School and High School Offices. Be sure to download or pick up the forms before the physical date since part of the form must be completed by your parents prior to the physical exam. The following schedule will be followed: 7:00am - Junior High 7:45am - Freshman 8:15am - Sophomores 8:45am - Juniors 9:15am - Seniors There will be a fee of $5.00 that will be collected on the day of the physical 2007-2008 OHSAA Physical Evaluation form |
