The Fifth Grade Behavior Plan
In an attempt to create classrooms where all students can be successful, the fifth grade team has established the following behavior plan that clearly outlines rules, consequences, and rewards for students in the fifth grade.
1.
All school rules apply.
2.
Respect other people and their property.
3.
Listen to others.
4.
Be prepared for class with all necessary materials
(homework, agenda, supplies, etc).
Listed below are the consequences that WILL
occur if any of these rules are broken.
The
consequences in this behavior plan are cumulative throughout the school year and
are in effect from classroom to classroom.
The consequences are:
Step 1
Warning (note sent home)
Step 2
Noon Intervention (note sent home)
Step 3
Noon Intervention (call to parents)
Step 4
Noon Intervention (team conference with the student)
Step 5
After School Intervention (parent conference)
Step 6
After School Intervention (team conference with the student &
principal)
Step 7
Office Referral
**Each additional step
from here will be an office referral.
**Steps in the plan may
be skipped depending on the severity of the behavior.
NOON
INTERVENTION During
the noon intervention, the student will NOT be allowed to work on homework.
He/She will be answering a series of questions regarding the behavior to
reflect on what happened. If a
student doesn’t show up, or refuses to do the intervention, he/she
automatically moves to the next step. Later
in the same day, the student will meet with a staff member to discuss the
situation/behavior to outline future plans to avoid similar situations. A note
will be sent home explaining the circumstances/behavior that warranted the
assignment of a noon intervention.
AFTER
SCHOOL INTERVENTION During
an after school intervention, students will NOT be allowed to work on their
homework. Instead students will
work through an activity that will allow them to examine the specific behavior
for which they were assigned the intervention. If a student doesn’t show up,
refuses to do the intervention, or is disrespectful to the monitor, he/she
automatically moves to the next step. These
interventions will be run once a week on one particular day.
OFFICE REFERRAL When a student is referred to the office, he/she will meet with Ms.
Kozarec. During this meeting the student may be assigned any of the following:
lunch detention, morning or after school detention, Saturday school,
suspension, or other consequences as deemed appropriate.
REWARDS
A majority of the rewards students receive for appropriate behavior will
occur through oral praise on a daily basis.
“Caught Being Good” awards will also be given to recognize
appropriate behavior via the system in place through the elementary office.
At the end of each nine weeks students who display appropriate behavior
by not progressing through the steps of the behavior plan will also be rewarded
with a special activity.
This activity might include a movie in the auditorium, pizza party, ice
cream party, etc.
